What it does

Why it matters
Answering "who has this patient already seen?" is a scavenger hunt in a standard EMR: a dozen tabs, several searches, and a patient who often cannot remember. Referrals go to the wrong specialist, duplicate work gets ordered, and the organization looks like it does not know its own patient.
